Hubungan Burnout Dengan Pengambilan Keputusan Etis oleh Perawat dalam End of Life Care di Ruang Intensif RSD Dr. Soebandi

Abstract

Background: Nurses in intensive care play a crucial role in ethical decisionmaking, especially in end-of-life care, due to their intensive interactions with critically ill patients and their families. The intensive care environment is characterized by high workloads, emotional stress, and recurring ethical dilemmas, which can lead to burnout. Burnout may reduce empathy, clarity of thought, and moral sensitivity, thereby affecting the quality of ethical decisionmaking. This study aimed to analyze the relationship between burnout and ethical decision-making among nurses providing end-of-life care in the intensive care unit at dr. Soebandi Hospital, Jember. Methods: This study employed a quantitative design with a correlational approach and a cross-sectional method. The sample consisted of 64 nurses working in the intensive care unit at dr. Soebandi Hospital, Jember, selected using a total sampling technique. Data were collected using the Maslach Burnout Inventory–Human Services Survey (MBIHSS) and the Nurses’ Ethical Decision-Making around End-of-Life Care Scale (NEDM-EOLCS). Data were analyzed using the Pearson Product Moment correlation test. Results: The results showed that the mean burnout score among nurses was 37.19 ± 7.81, while the mean ethical decision-making score was 123.70 ± 27.07. Pearson correlation analysis revealed a statistically significant moderate negative correlation between burnout and ethical decision-making (r = −0.509, p < 0.05). Conclusion: Burnout was significantly associated with nurses’ ethical decision-making abilities in intensive care units. Higher levels of burnout had the potential to reduce nurses’ ability to make ethical decisions in end-of-life care, indicating that burnout management was an important aspect of nursing practice in intensive care units.
