Analisis Risiko Kecelakaan Kerja pada PT. Blambangan Bahari Shipyard Menggunakan Metode FMEA dengan Rekomendasi ISO 45001 dan 5S.

Abstract

This research aims to analyze occupational safety risks at PT. Blambangan Bahari Shipyard using the Failure Mode and Effect Analysis (FMEA) method and to develop risk control recommendations based on ISO 45001:2018 and the 5S concept. The research focused on the fabrication and sub-assembly workshops, which have significant occupational hazards. Data were collected through observations, interviews, and questionnaires, then analyzed using the FMEA parameters of Severity (S), Occurrence (O), and Detection (D) to calculate the Risk Priority Number (RPN). The results identified 40 potential hazards, consisting of 30 low-risk, 2 medium-risk, and 8 high-risk categories. The dominant hazards were related to the repetitive use of electrical tools, exposure to fiberglass dust, workplace noise, welding activities, and electrical hazards. Based on these findings, risk control recommendations were developed using the ISO 45001:2018 hierarchy of controls and supported by the 5S concept through improved work discipline, proper use of personal protective equipment, workplace organization, and housekeeping. These recommendations are expected to reduce occupational safety risks and create a safer and more organized working environment.
