Abstract

Background: The implementation of Electronic Medical Records (EMR) has become a strategic priority in Indonesia's healthcare digital transformation and supports integration with the SATUSEHAT platform. However, evidence regarding factors influencing successful EMR implementation at district-level hospitals remains limited. This study aimed to analyze factors associated with the success of EMR implementation in hospitals in Jember Regency using the Consolidated Framework for Implementation Research (CFIR). Methods: This study employed a quantitative correlational design with a crosssectional approach. The research was conducted in three hospitals representing public, state-owned, and private healthcare institutions in Jember Regency. A total of 385 healthcare professionals, including physicians, nurses, pharmacists, nutritionists, physiotherapists, laboratory analysts, and radiographers, were selected using proportionate stratified random sampling. Data were collected using a CFIRbased questionnaire and analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM).. Results: The findings showed that all CFIR domains had positive relationships with successful EMR implementation. Innovation demonstrated a path coefficient of 0.039, outer setting 0.196, inner setting 0.045, individual characteristics 0.453, and implementation process 0.243. Among these variables, individual characteristics emerged as the most dominant factor influencing successful EMR implementation. The results indicate that healthcare professionals’ knowledge, motivation, selfefficacy, digital competence, and willingness to adapt significantly contribute to the effectiveness of EMR adoption. Conclusion: Successful EMR implementation in hospitals is influenced by innovation, outer setting, inner setting, individual characteristics, and implementation processes. Individual characteristics are the most influential determinant, highlighting the importance of strengthening healthcare workers’ readiness and digital competencies to support sustainable EMR implementation. Although technological innovation, organizational environment, and external support contributed to implementation success, their effects were relatively weak. The findings emphasize that successful digital transformation depends largely on human factors and organizational readiness. Continuous training, technical assistance, and strengthening digital competencies are essential to improve EMR utilization and sustainability.
