high risk for the worker especially for health and finally it can effect to decreased of
performance and productivity. Health risk management is needed for disease and accident
prevention.in occupational health. The objective of the research to apply health risk
management in Insecticide producton in PT Petrosida Gresik. Health risk management is
contain of identify, analysis and manage health hazard in PT Petrosida Gresik. Participative
observation is the methode of this research to get data for identify, analysis and manage
health hazard in PT Petrosida Gresik. Quantitative Risk Analysis Matrix-Level Of Risk The
Australia Standards / New Zealand Standards 4360, 2004 is used to calculating and
tabulating data. The result showed there are 63 risk factors with the spesification high risk
26 sources, medium risk 35 sources and low risk 2 sources and to manage the sources we can
