Gambaran Implementasi Keperawatan pada Pasien Cedera Otak dengan Masalah Risiko Perfusi Serebral Tidak Efektif yang Dirawat Inap di RSD Dr. Soebandi

Abstract

Traumatic brain injury is a major health problem that may increase the risk of ineffective cerebral tissue perfusion due to cerebral edema and elevated intracranial pressure. Therefore, appropriate nursing interventions are essential to maintain cerebral perfusion and prevent complications. This study aimed to describe nursing interventions in patients with traumatic brain injury at risk for ineffective cerebral tissue perfusion hospitalized at Dr. Soebandi Regional Hospital. This study used a retrospective descriptive design involving 113 medical records of patients hospitalized in 2025. Samples were selected using purposive sampling. Data were collected using a nursing intervention checklist based on the Indonesian Nursing Intervention Standards (SIKI) and analyzed using univariate analysis. The results showed that observational nursing interventions mainly involved monitoring signs and symptoms of increased intracranial pressure and respiratory status (100%). Therapeutic interventions comprised semi-Fowler positioning (100%) and oxygen therapy (71.7%). In the educational aspect, explaining the purpose and procedure of monitoring was implemented in 1.8% of patients. Collaborative interventions included anticonvulsant administration (19.5%) and osmotic diuretic administration (13.3%). The findings revealed that observational and therapeutic interventions were implemented more frequently than educational and collaborative interventions, providing a comprehensive overview of nursing implementation in patients with traumatic brain injury while highlighting the need to improve educational and collaborative interventions to enhance the quality of nursing care.
